Patient Assessment

We initially perform an assessment on our patients to assess their pharmacy requirements so Pharmacy Care Matters can provide a pharmacy service that best meets the needs of the patient.  The patient's needs are at the centre of the assessment.  The service user is regularly reviewed to ensure we are continuing to meet their pharmacy needs.


We work with healthcare professionals and where appropriate we also involve carers/ family members.

 

Prescription Reminder, Order and Collection

We remind our patients when their repeat prescription requires re-ordering so they do not run out of their medication. We order and collect repeat prescriptions for our patients from their doctor’s surgeries.  Alternatively, we can collect the prescription from the patient’s home or they can post the prescription to Pharmacy Care Matters.

Prescriptions are clinically checked

Our pharmacist clinically checks every prescription for correct dosages, drug interactions, administration instructions, correct formulation etc.  If the pharmacist has any concerns he will contact the doctor to ensure the medicines are safe for the patient and inform the patient of any changes to treatment.  The pharmacist will advise the patient of any special administration instructions.  This will reduce side effects and ensure the patient obtains most benefit from the prescribed medicine.

Dispense Medicines

We will dispense medicines in a way that best meets the needs of the patient:

  • In conventional medicine bottles and cartons.
  • In a monitored dosage system (MDS), which help patients remember when to take their medicines.  We can also put the patients picture on the label.

       

  • If carers administer the patients medicines we will dispense the medicines in a monitored dosage system (MDS) and supply MAR charts which are colour coded so it is easy to identify when the medicine is to be administered.  It also minimises any administration errors by carers.
 
  • We can also supply an MDS with detachable blisters so if you are going out you don’t have to take all your medicines with you, but just detach the number of blisters required and simply pop them in your bag.
  • We offer an Automated Pill Dispenser (to purchase) which sounds an alarm when it is time for you to take your medicines. We will collect, dispense and deliver it to your home for free.
  • We will also soon be introducing a Liquid Monitored Dosage System.

                            

Medicines Delivered to Patients

The medicines will be delivered to the patient’s home.  The pharmacist or nurse will deliver the medicines to the patient if they require special instructions or a demonstration on how to use a prescribed device.

NHS Electronic Prescription Service

To manage your repeat prescription more efficiently, Pharmacy Care Matters will be offering the NHS Electronic Prescription Service. This new service gives you the option to ‘nominate’ a pharmacy so that they can receive your prescriptions electronically. This service is reliable, secure and confidential. You can change or withdraw your nomination at any time at any pharmacy or at your doctor’s surgery.

7 Day Pharmacy Service

Pharmacy Care Matters provides a 7-day service every week of the year.

Medicines Advice

The patient can contact our pharmacist for advice on their medicines.

Medicines Use Review (medicines check-up)

The pharmacist will discuss with you your medicines to make sure you are obtaining optimum benefit from your medicines. It will give you the opportunity to learn more about your medicines, ask any questions or raise any concerns. In addition, the pharmacist will synchronise all medicines so they all start/ run out at the same time.

New Medicines Service

If you have been given a new medicine by your doctor to treat high blood pressure, type 2 diabetes, asthma/ chronic pulmonary disease or thin your blood then our pharmacist can help you get the best from your medicine. Our pharmacist will review you to see how you are getting on with your new medicine and answer any questions.

NHS Repeat Dispensing

We can dispense NHS repeat prescriptions issued by your doctor.

Collect Pharmacy Waste

Pharmacy Care Matters will collect all pharmacy waste from the patient’s home.

Free Medication Training for Carers

We understand Home Carers have a need for training. We provide our patients Home Carers with an on-going medicines training.
