Easy Monthly Process

We have a 28 day supply cycle.  Three weeks before your next medication cycle is due we will send you a Confirmation Sheet, which lists each service users current medicines.  This allows you to update our medication records and confirm what you want to order.  We collect the prescriptions from the doctor’s surgery or the care home.  Once we receive the Confirmation Sheets we check them against the prescriptions to ensure the right items have been prescribed. We also tell you what is missing and what has been changed well in advance and help rectify any issues before the monthly supply is due.

Prescriptions are clinically checked

Every prescription is clinically checked by the pharmacist for correct dosages, correct formulation, drug interactions etc.  If the pharmacist has any concerns he will contact the doctor to ensure the medicines are safe for the care home’s Service User and inform the care home of any changes to treatment.  The pharmacist will advise the care home staff of any special administration instructions.  This will reduce side effects and ensure the Service User obtains most benefit from the prescribed medicine.

Medicines are dispensed in a monitored dosage system (MDS) with the Service Users picture, which greatly minimises any administration errors by care home staff.

        

The MAR charts are colour coded and specially laid out so it is easy to identify medicines and what time they are due and each MAR chart is printed with the Service Users picture, which greatly minimises any administration errors by care home staff.  The labels state the manufacturer’s batch number and expiry date so there is a clear audit trail.



We will also soon be introducing a Liquid Monitored Dosage System.

                            

Medicines Delivered to the Care Home and Easy Check-In of Medicines


Medicines will be delivered in Pharmacy Care Matters' vehicles. Drivers wear plain uniforms and carry photo-id cards.  Our drivers are trained to understand the unique nature in which they work and to understand the needs of care homes to whom they are delivering.

We understand how much time checking-in the monthly medication can take.  That’s why we have developed special paperwork to accompany your monthly supply.  The paperwork that accompanies your monthly delivery is specially designed to help you check in the medication supply quickly and accurately.  Even the MAR charts are specially laid out to help you find items quickly and reduce the risk of errors.

7 Day Pharmacy Service

Medicines Use Review (medicines check-up)

The pharmacist will discuss with your Service User and their carer their medicines to make sure they are obtaining optimum benefit from their medicines.  It will give your Service User and their carer the opportunity to learn more about their medicines, ask any questions or raise any concerns.

New Medicines Service

If your Service User has been given a new medicine by their doctor to treat high blood pressure, type 2 diabetes, asthma/ chronic pulmonary disease or thin their blood then our pharmacist can help them get the best from their medicine.  Our pharmacist will review your Service User to see how they are getting on with their new medicine and answer any questions. 

Medicines Advice

Care home staff can contact our pharmacist’s for advice on all pharmacy queries.  

Pharmacy Waste

Pharmacy Care Matters will collect all pharmacy waste from the care home.

Free Medication Training and Medication Audits

We provide our care homes with an on-going medicines training.  We understand care homes have on-going need for training for existing and new staff.

We will regularly visit the care home to advice on medication handling, storage and administration to ensure you meet high standards.
